Good Faith Estimate:

As of January 1, 2022, Under Section 2799B-6 of the Public Health Service Act, if you’re uninsured or you pay for health care bills yourself (i.e. you do not submit claims to your health insurance carrier), we will provide you with an estimate of expected charges before you receive services. This is called a “good faith estimate.” Your provider will provide you a good faith estimate if you request one after services are scheduled. It will include expected charges for the primary service you’re receiving and any other items or services that are provided as part of the same scheduled experience.

Record Requests

We are required to retain your records for 7 years, or 5 years after a former child client has turned 18. If needed, records can be requested via your therapist. Please call or email them directly.Administrative charges may apply.
